Thank you for your participation in this survey on the experiences of recovering sex and pornography addicts who have participated in a fidelity polygraph and/or a professionally guided disclosure.

The FIRST PART of this survey will ask you questions about your participation in a FIDELITY POLYGRAPH if applicable and the SECOND PART of the survey will ask you questions about your participation in a PROFESSIONALLY GUIDED DISCLOSURE if applicable. Use the 'does not apply' option to rapidly move through the survey.

Your words of encouragement and advise are the most important part of the survey. They will be used to help in the individual and relationship recoveries of other addicts going through a fidelity polygraph or professionally guided disclosure process.
